Children’s Care Management
Children’s Care Management Services are available for children living in Jefferson, Lewis, and St. Lawrence Counties who have Medicaid that are experiencing emotional or behavioral difficulties. Services include: strength-based assessments, individualized service planning, culturally appropriate person and family center services, and transition planning, as needed.
