Children’s Care Management Services are available for children living in Jefferson, Lewis, and St. Lawrence Counties who have Medicaid that are experiencing emotional or behavioral difficulties. Services include: strength-based assessments, individualized service planning, culturally appropriate person and family centered services, and transition planning, as needed. The purpose of Children’s Care Management is to reduce the risk of long-term inpatient treatment, minimize use of emergency services, as well as to link children, youth, and their families to community based and support based services that are appropriate to their needs.
Children’s Care Management is best suited for youth under the age of 18 who:
- Receive Medicaid, Medicaid Managed Care, or are Medicaid Eligible
- Can Reside in Jefferson, Lewis and St Lawrence County
- Eligibility Determined Upon Specific Medical, Mental Health, and Complex Trauma Conditions
